Mundy Award Nomination

The Mundy award is designed to bestow distinction on an individual who has made an exceptional contribution to the field of adaptive seating and mobility. The presentation of this award is also designed to enhance public understanding of the role of rehabilitation professionals in Canadian society. The Mundy Award is presented during the annual Canadian Seating & Mobility Conference (CSMC).

Eligibility and Nominations
To be eligible for nomination, a candidate must be Canadian citizen or hold landed immigrant status and have been a resident of Canada for the past two years. The Call for Nominations will be published by CSMC each year. Nominations must be forwarded to the CSMC office by September 8th of that year. Nominations will then be submitted to an ad hoc Awards Committee for consideration. Nominations may be submitted by any member of the association or by any member of the public. There is no limit to the number of nominations that may be submitted. An award is not necessarily presented every year.Selection Criteria
Any exceptional contribution, either in Canada or on behalf of Canada, to the field of adaptive seating and mobility will be considered. Candidates are rated on the following criteria:- Work Related Achievements - practice, teaching, or research related to adaptive seating and mobility
- Service to the Profession - public education and understanding the role of adaptive seating and positioning and related rehabilitation technology in Canadian society, active participation in rehabilitation associations, societies, institutes, etc.
Nomination Documentation
Any individual wishing to make a nomination must complete the application form. Additional information describing achievements which may further support the nominee are welcomed.Nomination form requests the following information:
- Name and complete address of person(s) submitting this nomination including home and work address, phone/fax numbers and email as applicable
- Name and complete address of nominee including home and work address, phone/fax numbers and email as applicable
- Declaration of Nominee's Canadian Citizenship or landed immigrant status, and residency.
- Summary of significant work related achievements.
Include work related achievements in practice, teaching, or research related to adaptive seating and mobility - Summary of service to the profession
Include achievements related to public education and understanding of the role of adaptive seating, positioning and related rehabilitation technology in Canadian society, active participation in rehabilitation associations, societies, institutes, etc. - Other personal and general information
Provide other relevant information which may be of assistance in determining contributions including education, publications, directorships, honours and decorations, and any other relevant information
